Grupos controle de ensaios clínicos sobre intervenções não medicamentosas para dor oncológica: uma meta-pesquisa

A dor oncológica é um desafio comum em pacientes com câncer, originada da progressão tumoral ou dos efeitos adversos dos tratamentos. Para seu manejo, intervenções não-farmacológicas têm sido estudadas por ensaios clínicos randomizados (ECR), que requerem estratégias comparativas adequadas (placebo ou sham) para isolar os efeitos da intervenção. Entretanto, a limitação do mascaramento nesse tipo de intervenção pode influenciar a interpretação dos resultados. O objetivo deste estudo foi identificar as estratégias utilizadas nos grupos controle de ensaios clínicos randomizados que avaliam intervenções não-farmacológicas para a dor oncológica. Trata-se de uma meta pesquisa (investigação sobre como a ciência é produzida) nas bases MEDLINE e LILACS, utilizando descritores MeSH/DeCS e termos livres relacionados a “Cancer pain” e “Complementary therapies”, sem restrição de idioma ou ano de publicação. Excluiu-se estudos não randomizados e aqueles que envolviam intervenções cirúrgicas. A seleção e a extração dos dados ocorreram em duplicata. Foram extraídos informações sobre tipo de mascaramento e estratégias utilizadas nos grupos controle. A busca recuperou 311 artigos, 52 foram incluídos após a seleção, dos quais 22 apresentaram texto completo disponível. Destes, 15 eram simples-cegos, 6 abertos, 1 descrito como duplo-cego e 12 discutiram o impacto do viés de performance. As intervenções mais avaliadas incluíram acupuntura, massagem terapêutica e musicoterapia. Os grupos controle variaram entre tratamento usual, placebo, ausência de intervenção e procedimentos simulados. Conclui-se que há grande variabilidade nas estratégias de controle, e que a ausência de discussão sobre viés de performance pode comprometer a interpretação dos resultados e a tomada de decisão clínica.
